• $15.00 Cash or check
• A copy of your ID
• A printout of your FCC Registration Number (FRN)
• A printed copy of your FCC Form 605
• A copy of your FCC license and any CSCEs that apply
• A black pen and two pencils.  A basic calculator with a clear memory may also be used
